Peak Nursing Care provides professional, in-home clinical services for individuals with medical needs that require licensed oversight. Unlike many personal care companies, Peak is also a licensed home health agency, allowing us to deliver skilled services directly through our own nurses and therapists—without outsourcing or third-party partnerships.

Depending on eligibility and physician orders, nursing care may include:

• Comprehensive nursing assessments
• Ongoing clinical monitoring and symptom management
• Medication education and oversight
• Wound and skin care
• Post-hospital or post-surgical follow-up
• Chronic condition management (such as CHF, COPD, diabetes)
• Patient and caregiver education
• Coordination with physicians and care teams
   

Many non-medical home care companies must partner with outside organizations to arrange nursing services. Peak does not.

Because we are a licensed home health provider, we can:

• Use our own licensed nurses and therapists
• Coordinate care under one organization
• Reduce delays and communication gaps
• Provide smoother transitions between service levels
• Deliver truly integrated care at home
   

This means better continuity, clearer communication, and a more cohesive experience for patients and families.

Nursing services may be recommended for individuals who:

• Have recently been discharged from a hospital or rehab
• Have complex or changing medical conditions
• Require skilled oversight to avoid complications
• Need clinical education or monitoring at home
